NM_000162.5(GCK):c.1260G>T (p.Lys420Asn) was classified as Likely pathogenic for Maturity-onset diabetes of the young by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The p.K420N variant (also known as c.1260G>T), located in coding exon 10 of the GCK gene, results from a G to T substitution at nucleotide position 1260. The lysine at codon 420 is replaced by asparagine, an amino acid with similar properties. This variant has been identified in one individual with maturity-onset diabetes of the young (MODY) (Ambry internal data). A different alteration, located at the same position, p.K420E, has been detected in several individuals with MODY and has been shown to have mildly reduced enzymatic activity and lowered glucose affinity (Estalella I et al. Clin. Endocrinol. (Oxf), 2007 Oct;67:538-46; Estalella I et al. J. Hum. Genet., 2008 Mar;53:460-6; Capuano M et al. PLoS ONE, 2012 Jun;7:e38906). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the majority of available evidence to date, this variant is likely to be pathogenic.
