NM_001022.4(RPS19):c.72-9T>A was classified as Uncertain significance for Diamond-Blackfan anemia by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.72-9T>A intronic variant results from a T to A substitution 9 nucleotides upstream from coding exon 2 in the RPS19 gene. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. This nucleotide position is not well conserved in available vertebrate species on limited alignment. This alteration is predicted by the ESEfinder splice site tool to create a new alternate splice donor splice site. However, a different splice site prediction tool, BDGP, does not predict the creation of a non-native donor site, nor a deleterious effect on splicing and direct evidence is unavailable. Since supporting evidence is limited at this time, the clinical significance of this variant remains unclear.
