NM_000527.5(LDLR):c.665_679dup (p.Ser226_Asp227insGlyLysAspLysSer) was classified as Likely pathogenic for Cardiovascular phenotype by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.665_679dup15 variant (also known as p.S226_D227insGKDKS) is located in coding exon 4 of the LDLR gene. This variant results from an in-frame duplication of 15 nucleotides at nucleotide positions 665 to 679. This results in the insertion of five extra residues (glycine, lysine, aspartic acid, lysine, and serine) between codons 226 and 227, in the LDLR Class A repeat region. Based on internal structural analysis, this variant is anticipated to disrupt a known functional region of the protein (Jeon H and Blacklow C. Annu Rev Biochem 2005;74:535-62Pedersen NB et al. J Biol Chem. 2014;289(25):17312-24). In addition, this alteration is predicted to be deleterious by in silico analysis (Choi Y et al., PLoS ONE 2012; 7(10):e46688). Based on the majority of available evidence to date, this variant is likely to be pathogenic.
