Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000249.4(MLH1):c.601G>T (p.Val201Leu), citing Ambry Variant Classification Scheme 2023. This variant lies in the MLH1 gene (transcript NM_000249.4) at coding-DNA position 601, where G is replaced by T; at the protein level this means replaces valine at residue 201 with leucine — a missense variant. Submitter rationale: The p.V201L variant (also known as c.601G>T), located in coding exon 8 of the MLH1 gene, results from a G to T substitution at nucleotide position 601. The valine at codon 201 is replaced by leucine, an amino acid with highly similar properties. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. In addition, this alteration is predicted to be benign by MAPP-MMR in silico analyses (Chao EC et al. Hum. Mutat. 2008 Jun;29:852-60). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
