NM_024334.3(TMEM43):c.1201T>C (p.Ter401Arg) was classified as Uncertain significance for Cardiovascular phenotype by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.1201T>C variant (also known as p.*401REXT*11), located in coding exon 12 of the TMEM43 gene, results from a T to C substitution at nucleotide position 1201, which is the last nucleotide of the TMEM43 gene. The stop codon at position 401 is replaced by Arginine, resulting in an elongation of the protein by 11 amino acids. Frameshifts are typically deleterious in nature, however, this frameshift occurs at the 3' terminus of TMEM43, is not expected to trigger nonsense-mediated mRNA decay, and results in the elongation of the protein by 11 amino acids. The exact functional impact of these inserted amino acids is unknown at this time. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
