Likely pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_032043.3(BRIP1):c.507G>C (p.Gln169His), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRIP1 gene (transcript NM_032043.3) at coding-DNA position 507, where G is replaced by C; at the protein level this means replaces glutamine at residue 169 with histidine — a missense variant. Submitter rationale: The c.507G>C variant (also known as p.Q169H), located in coding exon 4 of the BRIP1 gene, results from a G to C substitution at nucleotide position 507. The amino acid change results in glutamine to histidine at codon 169, an amino acid with highly similar properties. However, this change occurs in the last base pair of coding exon 4, which makes it likely to have some effect on normal mRNA splicing. Another alteration impacting the same donor site (c.507G>A) has been detected in 4/706 cases with ovarian cancer, 1/6341 cases with breast cancer and in 0/36687 controls. RNA analysis of this alteration performed by the same group showed exon 5 skipping resulting in transcript that is 127 nucleotides shorter with a predicted alternate stop codon (p.S128*) (Weber-Lassalle N et al. Breast Cancer Res. 2018 Jan;20:7; Suszynska M et al. J Ovarian Res. 2020 May;13:50). The nucleotide and amino acid positions are highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site.
