Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_007194.4(CHEK2):c.487C>A (p.His163Asn), citing Ambry Variant Classification Scheme 2023. This variant lies in the CHEK2 gene (transcript NM_007194.4) at coding-DNA position 487, where C is replaced by A; at the protein level this means replaces histidine at residue 163 with asparagine — a missense variant. Submitter rationale: The p.H163N variant (also known as c.487C>A), located in coding exon 3 of the CHEK2 gene, results from a C to A substitution at nucleotide position 487. The histidine at codon 163 is replaced by asparagine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6502 samples (13004 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 72000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of p.H163N remains unclear.
