Uncertain significance for Primary ciliary dyskinesia — the classification assigned by Ambry Genetics to NM_001369.3(DNAH5):c.4861A>G (p.Ile1621Val), citing Ambry Variant Classification Scheme 2023. This variant lies in the DNAH5 gene (transcript NM_001369.3) at coding-DNA position 4861, where A is replaced by G; at the protein level this means replaces isoleucine at residue 1621 with valine — a missense variant. Submitter rationale: The p.I1621V variant (also known as c.4861A>G), located in coding exon 30 of the DNAH5 gene, results from an A to G substitution at nucleotide position 4861. The isoleucine at codon 1621 is replaced by valine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this variant remains unclear.
