Uncertain significance for Hereditary pulmonary alveolar proteinosis — the classification assigned by Ambry Genetics to NM_000542.5(SFTPB):c.370A>G (p.Ile124Val), citing Ambry Variant Classification Scheme 2023. This variant lies in the SFTPB gene (transcript NM_000542.5) at coding-DNA position 370, where A is replaced by G; at the protein level this means replaces isoleucine at residue 124 with valine — a missense variant. Submitter rationale: The p.I136V variant (also known as c.406A>G), located in coding exon 4 of the SFTPB gene, results from an A to G substitution at nucleotide position 406. The isoleucine at codon 136 is replaced by valine, an amino acid with highly similar properties. This amino acid position is well conserved in available vertebrate species; however, valine is the reference amino acid in other vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
