Likely pathogenic for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_001267550.2(TTN):c.66176dup (p.Cys22059fs), citing Ambry Variant Classification Scheme 2023: The c.38981dupG (p.C12994Wfs*2) alteration, located in exon 143 (coding exon 142) of the TTN gene, consists of a duplication of G at position 38981, causing a translational frameshift with a predicted alternate stop codon after 2 amino acids. This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This exon is located in the A-band region of the N2-B isoform of the titin protein and is constitutively expressed in TTN transcripts (percent spliced in or PSI 100%). Based on the available evidence, this alteration is classified as likely pathogenic.
