Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_004281.4(BAG3):c.326A>G (p.His109Arg), citing Ambry Variant Classification Scheme 2023. This variant lies in the BAG3 gene (transcript NM_004281.4) at coding-DNA position 326, where A is replaced by G; at the protein level this means replaces histidine at residue 109 with arginine — a missense variant. Submitter rationale: The p.H109R variant (also known as c.326A>G), located in coding exon 2 of the BAG3 gene, results from an A to G substitution at nucleotide position 326. The histidine at codon 109 is replaced by arginine, an amino acid with highly similar properties. This variant has been detected in an individual with dilated cardiomyopathy (Norton N et al. Am. J. Hum. Genet., 2011 Mar;88:273-82). This amino acid position is not well conserved in available vertebrate species, and arginine is the reference amino acid in other vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
