It has been assumed that this is a tandem duplication in direct orientation (Richardson_GIM_2018, Newman_AJHG_2015). Although exact breakpoints of this duplication are not known, it is expected to result in a large in-frame duplication change in the DPYD gene (amino acids 509-635), which would affect the first portion of the dihydroorotate dehydrogenase domain (amino acids 534-849; IPR005720). The variant was absent in 21694 control chromosomes (gnomAD database, Structural Variants dataset). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.(1524+1_1525-1)_(1905+1_1906-1)dup in individuals affected with Dihydropyrimidine Dehydrogenase Deficiency and no experimental evidence demonstrating its impact on protein function have been reported. One clinical diagnostic laboratory has submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation, classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.
