This variant lies in the ATM gene (transcript NM_000051.4) at coding-DNA position 4965, deleting one base; at the protein level this means shifts the reading frame starting at lysine residue 1656, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The c.4965delC pathogenic mutation, located in coding exon 32 of the ATM gene, results from a deletion of one nucleotide at nucleotide position 4965, causing a translational frameshift with a predicted alternate stop codon (p.K1656Rfs*4). This variant was detected in 1 of 5589 German BRCA1/2-negative probands with breast cancer (Hauke J et al. Cancer Med, 2018 Apr;7:1349-1358). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
