Uncertain significance for Carney-Stratakis syndrome; Paragangliomas with sensorineural hearing loss; Pheochromocytoma; Cowden syndrome 3 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_003002.4(SDHD):c.242C>G (p.Pro81Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces proline, which is neutral and non-polar, with arginine, which is basic and polar, at codon 81 of the SDHD protein (p.Pro81Arg). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with SDHD-related conditions. ClinVar contains an entry for this variant (Variation ID: 1687625). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on SDHD protein function. This variant disrupts the p.Pro81 amino acid residue in SDHD. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 10657297, 11391796, 11897817, 15479192, 19454582, 21348866, 21937622, 23433498, 24436918, 25494863). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
