NM_000261.2(MYOC):c.1279G>A (p.Ala427Thr) was classified as Uncertain Significance for Open-angle glaucoma by ClinGen Glaucoma Variant Curation Expert Panel, citing ClinGen Glaucoma ACMG Specifications V2.0.0 Approved. This variant lies in the MYOC gene (transcript NM_000261.2) at coding-DNA position 1279, where G is replaced by A; at the protein level this means replaces alanine at residue 427 with threonine — a missense variant. Submitter rationale: The c.1279G>A variant in MYOC is a missense variant predicted to cause substitution of Alanine by Threonine at amino acid 427 (p.Ala427Thr). The highest minor allele frequency of this variant was in the South Asian genetic ancestry group of gnomAD (v4.1.0) = 0.0001427 (13 alleles out of 91,084), which did not meet the PM2_Supporting allele frequency threshold (≤ 0.0001) or the BS1 allele frequency threshold (≥ 0.001). The REVEL score = 0.87, which was within the 0.773-0.931 range for PP3_Moderate, predicting a damaging effect on MYOC function. The Ala427Thr protein had similar secretion levels compared to wild type myocilin protein in this study (PMID: 16466712). The assay met the OddsPath threshold for BS3_Moderate (< 0.23), indicating that this variant did not impact protein function. This protein has also been assessed in another solubility/secretion assay (PMID: 36579626) as well as in thermostability assays (PMIDs: 24333014, 25524706, 23129764, 21612213, 36579626). Due to indeterminate and inconsistent results, the results from the other assays have not been included as functional evidence. Only 1 segregation had been reported for primary open angle glaucoma (PMID: 12189160), not meeting the ≥ 3 segregations required for PP1. Although probands with POAG have been reported carrying this variant, PM2_Supporting was not met, therefore PS4 did not apply. In summary, this variant met the criteria to receive a score of 0 and to be classified as a variant of uncertain significance (uncertain significance classification range -1 to 5, adapted from PMID: 32720330) for primary open angle glaucoma based on the ACMG/AMP criteria met, as specified by the ClinGen Glaucoma VCEP (v2.0.0, 5 Dec 2024): PP3_Moderate, BS3_Moderate
