NM_000157.4(GBA1):c.454+1G>A was classified as Uncertain significance for Lewy body dementia by Victorian Clinical Genetics Services, Murdoch Childrens Research Institute, citing ACMG Guidelines, 2015. This variant lies in the GBA1 gene (transcript NM_000157.4) at the canonical splice donor site of the intron immediately after coding-DNA position 454, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: Based on the classification scheme VCGS_Germline_v1.3.5, this variant is classified as VUS-3A. Following criteria are met: 0102 - Loss of function is a known mechanism of disease in this gene and is associated with Gaucher disease (MIM# 230800, 230900, 231000, 231005, 608013) and Lewy body dementia, susceptibility to (MIM#127750). (I) 0108 - This gene is associated with both recessive and dominant disease. Gaucher disease is an autosomal recessive condition; the different types are considered to fall within a spectrum, rather than distinct conditions, and are determined by age of disease onset and the presence and severity of neurologic function. Specific counseling about individual case prognosis is hindered by a significant overlap in the clinical features of individuals with various genotypes (OMIM, PMID: 20301446). Susceptibility to Lewy body dementia is associated with autosomal dominant inheritance. (I) 0115 - Variants in this gene are known to have variable expressivity. Gaucher disease is associated with marked clinical variability, even within the same family (PMID: 31010158). (I) 0211 - Canonical splice site variant without proven consequence on splicing (no functional evidence available). (SP) 0251 - This variant is heterozygous. (I) 0304 - Variant is present in gnomAD (v4) <0.01 (1 heterozygote, 0 homozygotes). (SP) 0505 - Abnormal splicing is predicted by in silico tools and affected nucleotide is highly conserved. (SP) 0704 - Another splice site variant comparable to the one identified in this case has limited previous evidence for pathogenicity. c.454+2T>A has been reported in an individual with early onset Parkinson disease (PMID: 37658046). (SP) 0803 - This variant has limited previous evidence of pathogenicity in an unrelated individual. This variant has been classified as likely pathogenic by a clinical laboratory in ClinVar. This variant has also possibly been observed in an individual with Gaucher disease type 1, however, it is not clear if this is the same variant as the transcript used is not specified (PMID: 17059888). (SP) 0905 - No published segregation evidence has been identified for this variant. (I) 1007 - No published functional evidence has been identified for this variant. (I) 1208 - Inheritance information for this variant is not currently available in this individual. (I) Legend: (SP) - Supporting pathogenic, (I) - Information, (SB) - Supporting benign
