This variant lies in the MRE11 gene (transcript NM_005591.4) at coding-DNA position 544, where G is replaced by A; at the protein level this means replaces glycine at residue 182 with arginine — a missense variant. Submitter rationale: The p.G182R variant (also known as c.544G>A), located in coding exon 5 of the MRE11A gene, results from a G to A substitution at nucleotide position 544. The amino acid change results in glycine to arginine at codon 182, an amino acid with dissimilar properties. This change occurs in the last base pair of coding exon 5, which makes it possible to have some effect on normal mRNA splicing. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6499 samples (12998 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.0005% (greater than 175000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to weaken the efficiency of the native splice donor site; however, direct evidence is unavailable. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.G182R remains unclear.
