Uncertain significance for Primary ciliary dyskinesia 23 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_018076.5(ODAD2):c.2842C>T (p.Arg948Cys), citing Invitae Variant Classification Sherloc (09022015): This variant has not been reported in the literature in individuals with ARMC4-related conditions. This variant is present in population databases (rs746173842, ExAC 0.006%). This sequence change replaces arginine with cysteine at codon 948 of the ARMC4 protein (p.Arg948Cys). The arginine residue is weakly conserved and there is a large physicochemical difference between arginine and cysteine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The cysteine amino acid residue is found in multiple mammalian species, suggesting that this missense change does not adversely affect protein function. These predictions have not been confirmed by published functional studies and their clinical significance is uncertain.
