Uncertain significance for Pityriasis rubra pilaris; Psoriasis 2 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001366385.1(CARD14):c.356T>A (p.Met119Lys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces methionine, which is neutral and non-polar, with lysine, which is basic and polar, at codon 119 of the CARD14 protein (p.Met119Lys). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with CARD14-related conditions. ClinVar contains an entry for this variant (Variation ID: 1678253). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt CARD14 protein function. This variant disrupts the p.Met119 amino acid residue in CARD14. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 29477734, 34004138, 34448248). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
