Pathogenic for Inborn genetic diseases — the classification assigned by Ambry Genetics to NM_001356.5(DDX3X):c.151+1G>A, citing Ambry Variant Classification Scheme 2023: The c.151+1G>A intronic variant results from a G to A substitution one nucleotide after coding exon 3 of the DDX3X gene. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This alteration was reported de novo in one individual with features consistent with DDX3X-related neurodevelopmental disorder (external communication 2024). Another alteration impacting the same donor site (c.151+1G>T) has been reported de novo in one individual from a cohort of individuals with intellectual disability (Lennox, 2020). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. Based on the available evidence, this alteration is classified as pathogenic.
