Pathogenic for Metachromatic leukodystrophy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000487.6(ARSA):c.1129_1136dup (p.Tyr381fs), citing Invitae Variant Classification Sherloc (09022015): This sequence change creates a premature translational stop signal (p.Tyr381Thrfs*45) in the ARSA gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 129 amino acid(s) of the ARSA protein. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with ARSA-related conditions. ClinVar contains an entry for this variant (Variation ID: 1675895). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This variant disrupts a region of the ARSA protein in which other variant(s) (p.Cys491Gly) have been determined to be pathogenic (PMID: 15026521; internal data). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
