Pathogenic for Charcot-Marie-Tooth Neuropathy X — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000166.6(GJB1):c.259C>G (p.Pro87Ala), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GJB1 gene (transcript NM_000166.6) at coding-DNA position 259, where C is replaced by G; at the protein level this means replaces proline at residue 87 with alanine — a missense variant. Submitter rationale: This sequence change replaces proline, which is neutral and non-polar, with alanine, which is neutral and non-polar, at codon 87 of the GJB1 protein (p.Pro87Ala). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with X-linked Charcot-Marie-Tooth disease (PMID: 8990008, 19062535, 25802885). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 155724). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt GJB1 protein function with a positive predictive value of 80%. Experimental studies are conflicting or provide insufficient evidence to determine the effect of this variant on GJB1 function (PMID: 9354338). For these reasons, this variant has been classified as Pathogenic.
