Likely pathogenic for Congenital contractural arachnodactyly — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001999.4(FBN2):c.4340G>A (p.Cys1447Tyr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the FBN2 gene (transcript NM_001999.4) at coding-DNA position 4340, where G is replaced by A; at the protein level this means replaces cysteine at residue 1447 with tyrosine — a missense variant. Submitter rationale: In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This variant affects a cysteine residue located within an epidermal growth factor (EGF)–like domain of the FBN2 protein. Cysteine residues in these domains are involved in the formation of disulfide bridges critical for protein structure and stability (PMID: 3495735, 4750422, 16677079). In addition, missense substitutions within the FBN2 EGF-like domains affecting cysteine residues are overrepresented in patients with congenital contractural arachnodactyly (PMID: 18767143). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt FBN2 protein function. This variant has not been reported in the literature in individuals with FBN2-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces cysteine with tyrosine at codon 1447 of the FBN2 protein (p.Cys1447Tyr). The cysteine residue is highly conserved and there is a large physicochemical difference between cysteine and tyrosine.
