Uncertain significance for Maple syrup urine disease, mild variant — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_152542.5(PPM1K):c.118A>C (p.Ser40Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces serine with arginine at codon 40 of the PPM1K protein (p.Ser40Arg). The serine residue is weakly conserved and there is a moderate physicochemical difference between serine and arginine. This variant is not present in population databases (ExAC no frequency). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. This variant has not been reported in the literature in individuals affected with PPM1K-related conditions.
