Uncertain significance for Muscular dystrophy-dystroglycanopathy (congenital with intellectual disability), type B2; Muscular dystrophy-dystroglycanopathy (congenital with brain and eye anomalies), type A2; Autosomal recessive limb-girdle muscular dystrophy type 2N — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_013382.7(POMT2):c.1621A>G (p.Ile541Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the POMT2 gene (transcript NM_013382.7) at coding-DNA position 1621, where A is replaced by G; at the protein level this means replaces isoleucine at residue 541 with valine — a missense variant. Submitter rationale: In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. This variant has not been reported in the literature in individuals affected with POMT2-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces isoleucine with valine at codon 541 of the POMT2 protein (p.Ile541Val). The isoleucine residue is highly conserved and there is a small physicochemical difference between isoleucine and valine.
