Uncertain significance for Dilated cardiomyopathy 1HH; Myofibrillar myopathy 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004281.4(BAG3):c.375G>T (p.Glu125Asp), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the BAG3 gene (transcript NM_004281.4) at coding-DNA position 375, where G is replaced by T; at the protein level this means replaces glutamic acid at residue 125 with aspartic acid — a missense variant. Submitter rationale: This sequence change replaces glutamic acid with aspartic acid at codon 125 of the BAG3 protein (p.Glu125Asp). The glutamic acid residue is highly conserved and there is a small physicochemical difference between glutamic acid and aspartic acid. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). This variant has not been reported in the literature in individuals with BAG3-related conditions. This variant is not present in population databases (ExAC no frequency).
