Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006946.4(SPTBN2):c.4360C>A (p.Gln1454Lys), citing Invitae Variant Classification Sherloc (09022015): This variant has not been reported in the literature in individuals affected with SPTBN2-related conditions. This sequence change replaces glutamine with lysine at codon 1454 of the SPTBN2 protein (p.Gln1454Lys). The glutamine residue is moderately conserved and there is a small physicochemical difference between glutamine and lysine. This variant is present in population databases (rs761287825, ExAC 0.01%). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt SPTBN2 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
