Uncertain significance for Leber congenital amaurosis 2; Retinitis pigmentosa 20 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000329.3(RPE65):c.1366G>C (p.Glu456Gln), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RPE65 gene (transcript NM_000329.3) at coding-DNA position 1366, where G is replaced by C; at the protein level this means replaces glutamic acid at residue 456 with glutamine — a missense variant. Submitter rationale: This sequence change replaces glutamic acid with glutamine at codon 456 of the RPE65 protein (p.Glu456Gln). The glutamic acid residue is highly conserved and there is a small physicochemical difference between glutamic acid and glutamine. This variant is present in population databases (rs755805126, ExAC 0.006%). This variant has not been reported in the literature in individuals affected with RPE65-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
