This variant lies in the POMT2 gene (transcript NM_013382.7) at coding-DNA position 2159, deleting one base; at the protein level this means shifts the reading frame starting at phenylalanine residue 720, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This premature translational stop signal has been observed in individual(s) with clinical features of muscular dystrophy-dystroglycanopathy (PMID: 22323514). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant disrupts a region of the POMT2 protein in which other variant(s) (p.Trp748Arg) have been observed in individuals with POMT2-related conditions (PMID: 17634419, 17869517). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Phe720Serfs*23) in the POMT2 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 31 amino acid(s) of the POMT2 protein.
