Uncertain significance for Familial hypobetalipoproteinemia 1; Hypercholesterolemia, autosomal dominant, type B — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000384.3(APOB):c.6957C>A (p.His2319Gln), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the APOB gene (transcript NM_000384.3) at coding-DNA position 6957, where C is replaced by A; at the protein level this means replaces histidine at residue 2319 with glutamine — a missense variant. Submitter rationale: Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant has not been reported in the literature in individuals with APOB-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces histidine with glutamine at codon 2319 of the APOB protein (p.His2319Gln). The histidine residue is weakly conserved and there is a small physicochemical difference between histidine and glutamine.
