Uncertain significance for Congenital myasthenic syndrome 17; Cenani-Lenz syndactyly syndrome; Sclerosteosis 2 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002334.4(LRP4):c.3344A>G (p.His1115Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces histidine, which is basic and polar, with arginine, which is basic and polar, at codon 1115 of the LRP4 protein (p.His1115Arg). This variant is present in population databases (rs758144252, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with LRP4-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
