Uncertain significance for Autosomal recessive limb-girdle muscular dystrophy type 2P; Muscular dystrophy-dystroglycanopathy (congenital with brain and eye anomalies), type A9 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004393.6(DAG1):c.409C>T (p.Arg137Trp), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the DAG1 gene (transcript NM_004393.6) at coding-DNA position 409, where C is replaced by T; at the protein level this means replaces arginine at residue 137 with tryptophan — a missense variant. Submitter rationale: This sequence change replaces arginine with tryptophan at codon 137 of the DAG1 protein (p.Arg137Trp). The arginine residue is moderately conserved and there is a moderate physicochemical difference between arginine and tryptophan. This variant is present in population databases (rs374440411, ExAC 0.01%). This variant has not been reported in the literature in individuals affected with DAG1-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
