Uncertain Significance for Severe combined immunodeficiency, autosomal recessive, T cell-negative, B cell-negative, NK cell-negative, due to adenosine deaminase deficiency — the classification assigned by ClinGen Severe Combined Immunodeficiency Variant Curation Expert Panel, ClinGen to NM_000022.4(ADA):c.935A>G (p.Lys312Arg), citing ClinGen SCID ACMG Specifications ADA V1.0.0. This variant lies in the ADA gene (transcript NM_000022.4) at coding-DNA position 935, where A is replaced by G; at the protein level this means replaces lysine at residue 312 with arginine — a missense variant. Submitter rationale: The c.935A>G (NM_000022.4) variant in ADA is a missense variant predicted to cause substitution of Lysine by Arginine at amino acid 312 (p.Lys312Arg). The filtering allele frequency (the upper threshold of the 95% CI of (4/418132 alleles) of the c.935A>G variant in ADA is 0.000003240 for European (non-Finnish) chromosomes by gnomAD v4, which is lower than the ClinGen SCID VCEP threshold (<0.0001742) for PM2_Supporting, and therefore meets this criterion (PM2_Supporting). No homozygotes have been observed in gnomAD. Another missense variant [c.934A>C, p.Lys312Gln] in the same codon has been reported; however, the ClinGen SCID VCEP classified this variant as VUS (PM5 not met). To our knowledge, this variant has not been reported in the literature in individuals affected with ADA-related conditions or in functional studies. Due to insufficient evidence, this variant is classified as a variant of uncertain significance for autosomal recessive SCID based on ACMG/AMP criteria applied, as specified by the ClinGen SCID VCEP (specification version 1.0): PM2_Supporting.
