Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_012193.4(FZD4):c.1490C>G (p.Ser497Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces serine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 497 of the FZD4 protein (p.Ser497Cys). This variant is present in population databases (rs80358300, gnomAD 0.004%). This variant has not been reported in the literature in individuals affected with FZD4-related conditions. ClinVar contains an entry for this variant (Variation ID: 1504097). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt FZD4 protein function with a negative predictive value of 80%. This variant disrupts the p.Ser497 amino acid residue in FZD4. Other variant(s) that disrupt this residue have been observed in individuals with FZD4-related conditions (PMID: 15223780, 38280677), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
