Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004183.4(BEST1):c.241G>T (p.Val81Leu), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 81 of the BEST1 protein (p.Val81Leu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with clinical features of autosomal dominant retinitis pigmentosa (Invitae). This variant has been reported in individual(s) with autosomal recessive bestrophinopathy (PMID: 31519547); however, the role of the variant in this condition is currently unclear. ClinVar contains an entry for this variant (Variation ID: 1501574). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt BEST1 protein function with a positive predictive value of 95%. This variant disrupts the p.Val81 amino acid residue in BEST1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 21109774, 26201355, 28687848, 31519547). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
