Uncertain significance for Epilepsy, childhood absence 4; Idiopathic generalized epilepsy; Epilepsy, idiopathic generalized, susceptibility to, 13 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001127644.2(GABRA1):c.560A>T (p.Tyr187Phe), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GABRA1 gene (transcript NM_001127644.2) at coding-DNA position 560, where A is replaced by T; at the protein level this means replaces tyrosine at residue 187 with phenylalanine — a missense variant. Submitter rationale: In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with GABRA1-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces tyrosine with phenylalanine at codon 187 of the GABRA1 protein (p.Tyr187Phe). The tyrosine residue is highly conserved and there is a small physicochemical difference between tyrosine and phenylalanine.
