Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_003002.4(SDHD):c.426C>G (p.Phe142Leu), citing Ambry Variant Classification Scheme 2023. This variant lies in the SDHD gene (transcript NM_003002.4) at coding-DNA position 426, where C is replaced by G; at the protein level this means replaces phenylalanine at residue 142 with leucine — a missense variant. Submitter rationale: The p.F142L variant (also known as c.426C>G), located in coding exon 4 of the SDHD gene, results from a C to G substitution at nucleotide position 426. The phenylalanine at codon 142 is replaced by leucine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6436 samples (12872 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.01% (greater than 10000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.F142L remains unclear.
