This variant lies in the NEB gene (transcript NM_001164508.2) at the canonical splice acceptor site of the intron immediately before coding-DNA position 295, where A is replaced by G; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.295-2A>G variant in NEB has not been previously reported in the literature in individuals with nemaline myopathy, but has been identified in 0.001% (1/74836) of African/African American chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP ID: rs1428300892). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 1497248) and has been interpreted as likely pathogenic by Invitae. This variant is located in the 3' splice region. Computational tools predict a splicing impact, though this information is not predictive enough to determine pathogenicity. There is an in-frame cryptic splice site 24 bases upstream from the intron-exon boundary. This is predicted to include a premature termination codon and lead to a truncated or absent protein. However this information is not predictive enough to determine pathogenicity. Loss of function of the NEB gene is an established disease mechanism in autosomal recessive nemaline myopathy. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive nemaline myopathy.
