Likely pathogenic for Neutropenia, severe congenital, 1, autosomal dominant; Cyclical neutropenia — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001972.4(ELANE):c.622T>G (p.Cys208Gly), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ELANE gene (transcript NM_001972.4) at coding-DNA position 622, where T is replaced by G; at the protein level this means replaces cysteine at residue 208 with glycine — a missense variant. Submitter rationale: This sequence change replaces cysteine with glycine at codon 208 of the ELANE protein (p.Cys208Gly). The cysteine residue is highly conserved and there is a large physicochemical difference between cysteine and glycine. This variant is not present in population databases (ExAC no frequency). This variant has been observed in individual(s) with neutropenia (PMID: 23463630, 25427142, Invitae). In at least one individual the variant was observed to be de novo. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
