NM_003721.4(RFXANK):c.436T>G (p.Trp146Gly) was classified as Uncertain significance for MHC class II deficiency by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces tryptophan with glycine at codon 146 of the RFXANK protein (p.Trp146Gly). The tryptophan residue is moderately conserved and there is a large physicochemical difference between tryptophan and glycine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with RFXANK-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain.
