NM_000431.4(MVK):c.418G>A (p.Gly140Arg) was classified as Uncertain significance for Mevalonic aciduria; Hyperimmunoglobulin D with periodic fever; Porokeratosis 3, disseminated superficial actinic type by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glycine with arginine at codon 140 of the MVK protein (p.Gly140Arg). The glycine residue is highly conserved and there is a moderate physicochemical difference between glycine and arginine. This variant is present in population databases (rs781108404, ExAC 0.002%). This variant has not been reported in the literature in individuals affected with MVK-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt MVK protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
