Uncertain significance for Joubert syndrome 23; Short-rib thoracic dysplasia 14 with polydactyly — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001329943.3(KIAA0586):c.4346A>T (p.Asp1449Val), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces aspartic acid, which is acidic and polar, with valine, which is neutral and non-polar, at codon 1502 of the KIAA0586 protein (p.Asp1502Val). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with KIAA0586-related conditions. ClinVar contains an entry for this variant (Variation ID: 1488369). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on KIAA0586 protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
