Uncertain significance for Catecholaminergic polymorphic ventricular tachycardia 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001035.3(RYR2):c.12458G>C (p.Ser4153Thr), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces serine with threonine at codon 4153 of the RYR2 protein (p.Ser4153Thr). The serine residue is highly conserved and there is a small physicochemical difference between serine and threonine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with RYR2-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt RYR2 protein function. This variant disrupts the p.Ser4153 amino acid residue in RYR2. Other variant(s) that disrupt this residue have been observed in individuals with RYR2-related conditions (PMID: 21652165, 26114861), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
