Uncertain significance for Pontocerebellar hypoplasia type 1A — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_003384.3(VRK1):c.1117A>G (p.Thr373Ala), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the VRK1 gene (transcript NM_003384.3) at coding-DNA position 1117, where A is replaced by G; at the protein level this means replaces threonine at residue 373 with alanine — a missense variant. Submitter rationale: This sequence change replaces threonine with alanine at codon 373 of the VRK1 protein (p.Thr373Ala). The threonine residue is weakly conserved and there is a small physicochemical difference between threonine and alanine. This variant is present in population databases (rs199759408, ExAC 0.01%). This variant has not been reported in the literature in individuals with VRK1-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
