Likely pathogenic for Hereditary insensitivity to pain with anhidrosis — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002529.4(NTRK1):c.3G>A (p.Met1Ile), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the NTRK1 gene (transcript NM_002529.4) at coding-DNA position 3, where G is replaced by A; at the protein level this means replaces methionine at residue 1 with isoleucine — a missense variant. Submitter rationale: This variant disrupts a region of the NTRK1 protein in which other variant(s) (p.Arg110Asp) have been observed in individuals with NTRK1-related conditions (PMID: 28328124). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. Disruption of the initiator codon has been observed in individual(s) with hereditary sensory and autonomic neuropathy (PMID: 32219930). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is not present in population databases (gnomAD no frequency). This sequence change affects the initiator methionine of the NTRK1 mRNA. The next in-frame methionine is located at codon 186. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
