Uncertain significance for Joubert syndrome 23; Short-rib thoracic dysplasia 14 with polydactyly — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001244189.2(KIAA0586):c.4T>C (p.Phe2Leu), citing Invitae Variant Classification Sherloc (09022015): This variant has not been reported in the literature in individuals with KIAA0586-related conditions. This sequence change replaces phenylalanine with leucine at codon 2 of the KIAA0586 protein (p.Phe2Leu). The phenylalanine residue is weakly conserved and there is a small physicochemical difference between phenylalanine and leucine. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate insufficient coverage at this position in the ExAC database. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
