Uncertain significance for 3-hydroxy-3-methylglutaryl-CoA synthase deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005518.4(HMGCS2):c.1451G>T (p.Ser484Ile), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces serine with isoleucine at codon 484 of the HMGCS2 protein (p.Ser484Ile). The serine residue is weakly conserved and there is a large physicochemical difference between serine and isoleucine. This variant is present in population databases (rs763005476, ExAC 0.009%). This variant has not been reported in the literature in individuals with HMGCS2-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
