Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_182746.3(MCM4):c.2297G>A (p.Arg766Gln), citing Invitae Variant Classification Sherloc (09022015): Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This sequence change replaces arginine with glutamine at codon 766 of the MCM4 protein (p.Arg766Gln). The arginine residue is highly conserved and there is a small physicochemical difference between arginine and glutamine. This variant is present in population databases (rs144284210, ExAC 0.01%). This variant has not been reported in the literature in individuals with MCM4-related conditions. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
