Uncertain significance for Neuronal ceroid lipofuscinosis 11; GRN-related frontotemporal lobar degeneration with Tdp43 inclusions — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002087.4(GRN):c.1373C>G (p.Pro458Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GRN gene (transcript NM_002087.4) at coding-DNA position 1373, where C is replaced by G; at the protein level this means replaces proline at residue 458 with arginine — a missense variant. Submitter rationale: Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). This variant has not been reported in the literature in individuals affected with GRN-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces proline with arginine at codon 458 of the GRN protein (p.Pro458Arg). The proline residue is moderately conserved and there is a moderate physicochemical difference between proline and arginine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
