Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_015909.4(NBAS):c.379G>A (p.Val127Ile), citing Invitae Variant Classification Sherloc (09022015): This variant has not been reported in the literature in individuals affected with NBAS-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces valine with isoleucine at codon 127 of the NBAS protein (p.Val127Ile). The valine residue is highly conserved and there is a small physicochemical difference between valine and isoleucine. This variant also falls at the last nucleotide of exon 6, which is part of the consensus splice site for this exon. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site.
